At Charlottesville Orthopaedic Center, we understand that healing from an injury or managing chronic joint pain can be frustrating. If you’re looking for a non-surgical, natural approach to support your recovery, Platelet Rich Plasma (PRP) Therapy might be the solution you’ve been searching for.

What Is PRP Therapy?

Platelet Rich Plasma, or PRP, therapy is a cutting-edge regenerative treatment that uses your body’s own healing power to support recovery from musculoskeletal injuries and conditions.

Your blood contains platelets, which are rich in growth factors that help tissue heal and regenerate. During a PRP treatment, we draw a small sample of your blood, process it in a centrifuge to concentrate the platelets, and then inject that concentrated solution into the area that needs healing, such as a joint, tendon, or ligament.

How PRP Supports Orthopedic Healing

PRP therapy works by enhancing the body’s natural healing process. The growth factors in platelets reduce inflammation, stimulate tissue repair, and support new cell growth. In orthopedics, this can mean faster healing and less pain.

Some of the most common orthopedic uses of PRP include:

  • Tendon injuries, like tennis elbow or Achilles tendinitis
  • Ligament sprains and strains
  • Joint pain, particularly in the knees, hips, and shoulders
  • Mild to moderate osteoarthritis
  • And many more

Many athletes and active individuals turn to PRP as a way to recover faster without surgery, and now, you can too.

Your PRP Consultation

At Charlottesville Orthopaedic Center, your PRP therapy begins with an in-depth consultation with Dr. Nielsen to determine if you’re a good candidate for the procedure. During this initial visit, we’ll review your medical, athletic, and occupational history, focusing on how your injury has impacted your daily life. This will help us understand the root cause of your pain or dysfunction.

The injured area will be thoroughly examined, and diagnostic imaging, often using ultrasound, will be performed to get a detailed look at the affected tissues. Ultrasound is the preferred imaging method for PRP injections, as it allows us to guide the injection precisely into the damaged area. If you’ve had previous imaging done (such as X-rays, MRIs, or CT scans), please bring those reports with you to your consultation. These images will help us assess your condition and develop the best treatment plan for your needs.

After the consultation and imaging, we’ll walk you through the specifics of the PRP procedure, explaining what to expect and answering any questions you might have.

What to Expect During Your PRP Treatment

Your PRP therapy is a simple, in-office procedure that typically takes less than an hour from start to finish. It begins with a personalized consultation where our team will assess your symptoms, review any necessary imaging, and determine if PRP is the right approach for your condition.

If you’re a good candidate, we’ll draw a small sample of your blood, usually from your arm, then use a centrifuge to separate the platelets from the other components. Once the platelet-rich plasma is ready, we will carefully inject it into the targeted area, such as a joint, tendon, or ligament. In many cases, we use ultrasound guidance to ensure precision and comfort.

After the injection, most patients are able to resume light activities the same day, although we may advise you to limit strenuous exercise or heavy lifting for a short period while your body begins the healing process. You can expect to start noticing gradual improvements in pain and function within a few weeks, with continued progress over the next few months as the regenerative effects take hold.

Benefits of PRP Therapy

  • Minimally invasive: No incisions or surgical downtime
  • Natural healing: Uses your own blood to stimulate recovery
  • Low risk: Since PRP is derived from your own body, allergic reactions are rare
  • Potential to delay or avoid surgery: Especially in early arthritis or chronic injuries

PRP isn’t a quick fix, but for many patients, it offers long-term improvement without relying solely on medication or invasive procedures.

Platelet Rich Plasma (PRP) Therapy FAQs

How long does platelet rich plasma last?

Depending on your condition, platelet-rich plasma results can last anywhere from a few months to a year or longer. Your provider will discuss what you can expect during your consultation.

Does PRP really work?

Yes. Much research has gone into the development of PRP-based procedures.

Who is NOT a candidate for PRP therapy?

Anyone with a cancer (such as prostate cancer or breast cancer), not in remission for at least 5 years rnrnCertain other malignancies or blood-borne diseases that you are being treated for rnrnAny current infection rnrnPatients using a high dosage of Coumadin rnrnGeneral procedural contraindication rnrnPatients with multiple medical issues may not be good candidates

What is the cost of PRP therapy?

The cost of PRP treatment may vary depending on the area being treated. Please contact us today for pricing information. Insurance companies, for the most part, will not pay for PRP. This is generally paid for out of pocket.

How long do I have to wait to return to normal activities?

How quickly you can return to your normal activities depends on the condition and body part being treated. rnrnInjections in the elbow or shoulder are placed in a sling for 48 hours to 1 week or more, and then as needed for comfort. rnrnFor the ankle (Achilles tendon, peroneal tendon, plantar fascia), you will be required to wear a walking boot for up to two weeks or more. (If the injection is on the right side, you will not be able to drive with the boot on.) rnrnIf you have an injection in your knee for osteoarthritis, you only need to rest for 24 hours, and then you can return to your normal activities. If the injection is in your patellar tendon or quadriceps tendon, your knee will be placed in a knee immobilizer for 1-2 days. rnrnAll procedures, except for possibly knee osteoarthritis, will begin therapy 1 week after the injection.

How long does it take for PRP therapy to “work”?

How long it takes the PRP to start working will depend on the number of injections you receive and the condition you’re struggling with. That said, most patients will notice an improvement in their pain or function within a few weeks. More regenerative effects can take a few months to become evident.

Are there any side effects or complications of PRP?

As with any treatment, there are some side effects and risks associated with PRP injections. Some of the most common side effects include discomfort at the injection site, bruising, and swelling. These side effects are temporary and will dissipate within a day or two.

Can I drive home after the PRP injection?

If the injection is in your right knee or ankle tendons, you should bring a driver. If the injection is located in any other body part, you may drive yourself home after the injection.

Can I take my normal medications before and after the injection?

Do not take anti-inflammatory medication such as Advil, Aleve, Motrin, Ibuprofen, Mobic, Naproxen, Nabumetone, or Diclofenac for 7 days prior to the injection and several weeks after the last injection. Pain medication will be prescribed the day of the procedure to be taken as needed. A regular aspirin regimen (81mg/day) is ok. Tylenol/Acetaminophen is allowed.

What is the “typical” response to PRP treatment?

Research and clinical experience suggest that PRP can successfully treat pain from chronic degeneration and tears of tissue tendons, and pain from mild to moderate joint articular cartilage degeneration. It is important to understand that PRP isn’t a miracle cure. The Hospital for Special Surgery in NYC estimates that their success rate for the platelet-rich plasma treatment, or PRP, is between 60 and 75 percent. (success=>50% relief of pain) rnrnThe Mayo Clinic claims an 80% success rate. However, for a minimally invasive procedure in injuries/conditions that are notoriously hard to heal, that’s pretty impressive. rnrnUltrasound guidance is essential to the accuracy of placement and enhancing the efficacy of the injection.

What if I don’t get a good enough response with PRP treatment?

Then bone marrow stem cell therapy or surgical treatment might be the next step.

Who is an ideal candidate for PRP Therapy?

If you are struggling with musculoskeletal injuries, have realistic expectations of treatment, and have not seen results from other treatments, you are likely a good candidate for PRP. At your consultation, we’ll review your medical history, discuss your concerns, and then determine if PRP injections are right for you.

Dr. Nielsen and his team are great! I’m relatively new to the Charlottesville area and have been very frustrated by the long wait times for medical appointments and then the brevity of those appointments once they finally arrive. Dr. Nielsen and his team are the opposite. They go above and beyond spending more than ample time with you, clearly explaining their diagnosis and answering any questions you may have. I highly recommend them.

I cannot say enough great things about Charlottesville Orthopaedic Center! I fell and broke my wrist, called my PC and they suggested them. Natalie was awesome! The first broken bone I have ever had and I was quite nervous. The techs were great with X-rays and fitting me with a cast. After a few months of appointments I am on the road to recovery!! The ladies at the front desk are always friendly and make me feel welcome too. It is nice to know of such a great practice here in Charlottesville. Kudos!!
