Telemedicine Consent
The U.S. Department of Health and Human Services defines telehealth (telemedicine) as the use of electronic information and telecommunications technologies to support and promote long-distance clinical health care. Charlottesville Orthopaedic Center uses telehealth technologies in various capacities to care for patients when face to face encounters are not the best option. There are limitations to these visits and possible electronic problems beyond the control of the practice or patient. Protected Health Information for telemedicine visits will be safeguarded using approved transmission methods. I understand the general concept of telemedicine and can/will ask any questions about how it will be used to meet my healthcare needs. By agreeing to the TERMS OF SERVICE and entering into the virtual waiting room, I consent to the use of telemedicine for medical care at Charlottesville Orthopaedic Center.
